The agreement for the Home Study must also make provisions for three post-placement reports at 3, 6 and 12 months after adoption has taken place. A copy of the post-placement agreement (see Appendix) needs to be prepared. This document only needs to be notarized (not state certified and authenticated) and sent to CHI. Do not attach this document to the Home Study.

I. The following is just an example of what your home study agency might request from you. Please follow the requirements of your home study agency.

  1. An autobiography about yourself (2 page summary)

    • Personal history

    • Education

    • Marital relationship

    • Employment and Finances

    • Description of home and neighborhood

    • Motivation to adopt

    • Experience with children and attitudes about discipline

    • Support networks. Will family and friends be supportive?

  2. Photocopies: Birth Certificates for each family member.

  3. Photocopies: Marriage Certificate.

  4. Photocopies: Divorce Decree, if applicable.

  5. Copy of letter from local police saying you have no record.

  6. Copy of your Employment/Financial Statement.

  7. Copy of medical from physician showing good mental/physical health and statement from doctor for adoptive parents all children living in the home.

  8. Three reference letters, from non-family members (copies can be sent to Children’s Hope International).

  9. Copy of latest 1040 tax return.

II. Please ask your social worker or send yourself: a copy of the home study, agency license, the home study/post placement agreement and three reference letters to Children’s Hope.
