Dossier Check List

Prepare or collect all of the following documents. These documents must be originals. Your adoption consultant will be continually available to direct you through this process.

I. THESE ITEMS ALL REQUIRE NOTARIZATION, STATE CERTIFICATION AND AUTHENTICATION BY THE U.S. DEPARTMENT OF STATE AND THE EMBASSY OF ETHIOPIA

¨ CHI Agency License (CHI will submit with dossier)

¨ Letter stating reason for adopting from Ethiopia

¨ Photos of applicant(s) and home (6-8)

¨ CIS approval

¨ Home Study (less than 6 months when submitted to CIS)

¨ Home Study Agency License

¨ Letter of Commitment for Post Adoption – Social Worker

¨ Employment Letter(s)

¨ Financial Statement

¨ Birth Certificate(s)

¨ Marriage Certificate/ Divorce Decree or Single Statement

¨ Medical letter(s)

¨ Police letter(s)

¨ Three (3) signed reference letters from non-relatives

¨ Letter of Obligation for Post Adoption - Family

¨ Qualification for Naturalization

¨ Passport ID pages

• Special Power of Attorney – for 1 parent traveling – This document is required when ready to travel to Ethiopia. Original to be carried by the traveling spouse.

II. ITEMS NOT NEEDING NOTARIZATION, CERTIFICATION OR AUTHENTICATION, BUT NEEDED TO COMPLETE DOSSIER

¨ Passport photos – 4 (2 sets) for each parent.
**Keep one set (2) with you to use for your travel visa and send another set to CHI with your dossier.

III. (FOR CHI FILES) – PLEASE SEND COPIES OF THE FOLLOWING DOCUMENTS TO CHI UPON COMPLETION OF HOME STUDY
